In booklet format (no p. nos., but note that the scans nos. 2-3 are repeated as scans nos. 4-5, and so on, since all the scans are doubled except the very last one), and one of the few Matho mss. that is not fragmentary but complete (but no, the end is missing!), this is definitely a Zhi byed text with mention of Dam pa Rgya gar ba. It can be seen to belong to his Middle Lineages, more specifically the Skam lugs. (Dan Martin's Hurried Notes)
